  • treatment for chronic pain, opioid use disorder (OUD), and alcohol use disorder (AUD)

By appointment only, call ahead 
Mon 9 am-4 pm, Tue 9 am-6 pm, Wed 8 am-1 pm, Thu 8 am-5 pm, Fri 8 am-3 pm (closed daily for lunch 12:30 pm-1:45 pm)

People with opioid use disorder (OUD), alcohol use disorder (AUD), pain and the comorbidity of mental health
Application
  • medical referral required for pain
  • self-referrals accepted for opioid use disorder (OUD) or alcohol use disorder (AUD)
  • physicians will see patients with dependency/OUD issues the same day or the next scheduled day in most cases
