Description and Service Details

Description (Service)	Free drug plan with no drug costs, no deductibles, no copays. $1000 per person for 12 months from the enrolment date (can reapply). Medications eligible under the program are delivered to your home address or closest Canada Post location free of charge. The list of medications covered under the program is comprehensive and is considered an 'essential list'. Some drugs may require pre-approval (i.e., brand name medications).

Eligibility	A resident of Ontario Ages of 25 - 64 Currently employed Need or anticipate the need for prescription medications to treat a medical condition Not enrolled in a private or employee prescription drug plan Not enrolled in, or eligible for, prescription drug coverage through a public program in Ontario Have an income meeting low-income criteria
Application	Contact the Program Specialist and complete application form A prescreen tool is available by visiting the GreenShield website here and clicking "Find out if you're eligible" • Individuals approved for the program will need to register an online account with the program's pharmacy to start receiving medications
