Description (Service) |
Offers treatment and management for individuals whose diabetes and related conditions require a more intensive and comprehensive team-based approach * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * team consists of a nurse practitioner, registered nurse, registered dietitian, social worker, and pharmacist- help to stabilize and help to control your diabetes and diabetes-related complications, such as chronic kidney disease, cardiovascular disease, diabetic retinopathy, neuropathy and related conditions
- prolonged diabetes and comorbidities such as organ failure, frequent hyper or hypoglycemia, foot and skin issues, mental health issues and inability to cope
- psychiatrist consultation
- wound care specialists
- assist in navigating health care system
- referral to diabetes education program or other related community-based support programs for ongoing support, as needed
