Description (Service) | COVID-19 Service Change: Attendant Outreach services priorities are: -single, with no or minimal family support, -complex care or
high care needs, -time specific services that can not change. No change to services in Supportive Housing. Housing and Attendant Services help adults live in their communities and fully participate in the activities of daily living * the service is non-medical and the service philosophy is to support independent, responsible living by the adult with a physical disability * volunteer opportunities available * consumers are typically persons with multiple sclerosis, ALS, cerebral palsy or disabled by various other progressive diseases or syndromes Deborah's Home

Eligibility | Deborah's Home: Clients must be age 16 or older, qualify for rent subsidy, require a wheelchair accessible living environment and be physically disabled Attendant Outreach: Persons with a physical disability age 16 or older * has a valid OHIP card * has the ability to direct their own personal support and homemaking services * residents of Halton (including Burlington) |
