Description and Service Details

Description (Service) Help adults live in their communities and fully participate in the activities of daily living * helps adults with disabilities to remain independent * volunteer opportunities available

Supportive housing (Deborah's Home):
  • residence for five adults with physical disabilities requiring 24 hour access to personal support services and homemaking support
  • tenants who wish to live at Dehorah's Home must be over the age of 16, require a wheelchair accessible living environment, qualified for Rent Geared to Income (RGI) housing, possess valid OHIP, and require access to personal and homemaking assistance 24/7

Attendant Outreach Services:
  • a service designed to meet the needs of adults with physical disabilities
  • personal and homemaking support is provided to eligible adults in their own homes, schools or workplaces in Halton Region
  • services include hygiene, grooming, positioning, dressing and undressing, assist with medication, bowel and bladder care, transfers, meals, housekeeping
Hours Attendant Outreach is available daily 6am-11pm

Eligibility Deborah's Home: Clients must be age 16 or older, qualify for rent subsidy, require a wheelchair accessible living environment and be physically disabled
Attendant Outreach: Persons with a physical disability age 16 or older * has a valid OHIP card * has the ability to direct their own personal support and homemaking services * residents of Halton (including Burlington)
Application Application and needs assessment forms can be downloaded from the website and completed or call the main office directly for an application
