Description and Service Details
Description (Service) Support coordinators provide support and services based on the person's level of need.  
Rent-geared-to-income supportive housing options: 
* transitional housing leading to independent living  
* collaborative shared housing  
* shared townhouse living  
* individual apartments 
Mental health community support programs: 
Staff assist with:  
* case management  
* advocacy  
* life skills training  
* supportive counselling  
* activities of daily living  
* access to community services  
* 24/7 access to telephone support for program participants 
* ongoing support and housing services to youth at risk ready to transition into the adult system 
Programs & services to support mental wellness (Consumer Survivor Initiatives): 
* a peer led and run initiative for people living with mental health AND substance use challenges 
* Anxiety, Self Esteem and Concurrent Disorders workshops are peer led programs 
* Wellness Recovery Action Plan (WRAP) workshops are available for individuals 
* Checkpoint is a peer led drop in session providing support in a group setting 
* harm reduction programs including Evolution supporting people who are living with mental health and substance use challenges

* be willing to contribute to a safe, secure healthy living space for all residents and /or neighbors 
* ability to live independently or have the appropriate supports in place to live independently
