Support Coordination for Adults 
* responds to the needs of adults who experience mental health and/or substance use issues in Halton 
* offer support coordination, housing, peer support, harm reduction, navigation, and distress/crisis support 
* supports include a Justice Team that specializes in supporting the unique needs of individuals that have mental health and/or substance use issues and are justice-involved 
 
Support Coordination for Youth 
* responds to the unique needs of youth, aged 16-24, in Halton 
* offer support coordination, housing, peer support, harm reduction, navigation, distress/crisis support, and support moving into adult services as needed 
 
TAY Service Coordination (Transitional Aged Youth) 
* multi-agency committee that problem solves and assists youth who may benefit from an enhanced service plan and to successfully bridge youth into adult services
