Helping to address questions about what to expect and how to manage Type 1 Diabetes (T1D) during the transition from secondary to post-secondary education, by working with students who have already transitioned with T1D to develop programs and resources that provide the information and support needed to successfully transition

  • offers free downloadable resources, informative webinars, and peer support programs
  • online transition guide, planner, webinars and hub

HOPE Connects

  • Peer Support Program to support students as they balance their T1D during their transition to post-secondary school
  • students can access information and support through a variety of forums
  • one to one peer mentorship and monthly virtual drop-ins

Free to Be

  • in-person program combines elements of visual and dramatic arts to embrace life with type 1 diabetes (T1D)
  • open to students in secondary school from across the GTA
  • youth have the opportunity to connect with fellow T1Ds and explore the day to day highs and lows of life with diabetes using visual arts, improv and classical theatre techniques

Online Zine Project (T1Zine)

  • a collective work created by youth from across Canada
  • participants use thought provoking visual arts and creative writing to express and educate

PEP Talks: Parents Empowering Parents

  • Transition Support for Parents & Caregivers of Youth Living with T1D in Halton and Peel regions
  • a four-part virtual workshop series focused on getting the tools, information and support needed to navigate the transition to post-secondary school and beyond
  • will give parents and caregivers the opportunity to find a community of voices to collaborate with as they prepare for their child’s transition from secondary to post-secondary education and from pediatric to adult diabetes care

Diabetes Hope Scholarship

  • a combined merit and needs based scholarship that celebrates the resiliency of youth with type 1 diabetes
  • awarded to students who exhibit excellence in balancing the many responsibilities associated with diabetes management while also overcoming personal challenges, maintaining strong academic standing, enriching their lives through community involvement and extra-curricular activities, and maintaining their health
