| Description (Service) | * issues marriage licences authorized officiants are available to perform simple, dignified civil marriage ceremonies * offers a Commissioner of Oaths authorized by the Province of Ontario, to take oaths or statutory declarations * offers Licensing Services * responds to Freedom of Information requests For a death that occurred in Ontario, apply online for a death certificate through the ServiceOntario website. Complete the death certificate application through that website, and submit it online or print a hard copy for mailing. A hard copy application is also available in person from the Clerk's department at Town Hall and mail it to: Province of Ontario Office of the Registrar General P.O.
