Description (Service) | The supports for daily living program offers on-site 24-hour personal support services for seniors. * located close to Milton Mall and Milton Seniors Centre Halton Region provides the Supports for Daily Living program to the residents of Martin House and eligible individuals living in the surrounding neighbourhood. Support can include, but is not limited to: * personal care including showering * dressing/undressing * assistance with pre-measured medication * reassurance * safety checks Those who qualify for personal care services may also be eligible for: * light housekeeping * laundry * assistance with meal services Other locations for Assisted Living/Supports for Daily Living Projects managed by the Region of Halton. |

Eligibility | Ages: 65 year(s) and up Designed to support high risk seniors, aged 65+ who have complex needs, and who are medically stable These services are currently available to individuals who: * live in their own private home, or within a residential setting such as an apartment building or senior citizens' residence * demonstrate a need via a Community Health Assessment for daily access to personal support services throughout a 24 hour period (with or without homemaking services) on a frequent basis * are able to direct their own care or have a substitute decision maker or a live-in caregiver to direct care * are able to communicate their needs (with or without aides) |
