Description (Service) | Community Residential Facility -- case management and correctional planning in a supportive living environment for men transitioning from federal correctional facilities New Leaf Program -- housing and supports for men coming off the streets or overcoming situations of homelessness due to acute mental illness and/or substance abuse Graduate Program -- supports clients 50 years or older who have successfully transitioned from either the New Leaf or Community Residential Facility program
