Description and Service Details

Description (Service)	Provides support to clients for high-risk transitions to long-term care homes for people who exhibit responsive behaviours transitional lead works with client's family and care providers to develop behavioural care plans as client is admitted into long-term care works with BSO Long-Term Care Home Mobile Team to provide intensive support in first six weeks after admission
Hours	Mon-Fri 7:30 am-3:30 pm
