Description (Service)	Free community support services coordinated by Ontario Health atHome to promote successful discharges from either in-patient hospital stays or emergency department visits provides individuals access to services for a 4-6 week period to regain strength and independence Services may include: frozen meal delivery transportation home from hospital transportation for 2 follow up medical appointments homemaking services home safety assessment wellness check ins communication with other support services access to a 24/7 support line
