Description (Service) | Referrals being sent to central location, triaged and reviewed by clinician and directed to appropriate services based on the patient care needs * individuals can initiate a referral for diabetes education * once the self-referral has been initiated, your family doctor or other health care provider will be contacted to complete the process and the referral will be sent to the diabetes program
