Housing First Program

• centers on first moving people experiencing homelessness into independent and permanent housing and then providing additional supports and services such as physical and mental health, education, employment, substance abuse and community connections

HOMES Program

• a community partnership initiative that provides supportive housing for adults who have been diagnosed with a serious mental illness who are homeless or at risk of becoming homeless
  provides rent supplement housing with dedicated supports throughout the Region of Halton
• Summit is the lead agency in a partnership with Joseph Brant Memorial Hospital, ADAPT, Support House and the North Halton Mental Health Clinic providing support
